Moxibustion for Dampness Pattern: Phlegm, Edema and Weight Management

Dampness Pattern (Shi Zheng, 湿证) in Chinese medicine refers to a state where pathogenic dampness obstructs the body fluids and qi movement. Dampness is heavy, turbid, and sticky – it tends to settle in the lower body and causes lingering, chronic symptoms. Moxibustion resolves dampness by warming the spleen to transform and transport fluids. This guide covers dampness subtypes and their protocols.

What is Dampness Pattern in Chinese medicine?

Dampness arises from environmental exposure (humidity, living in damp climates), dietary factors (excessive dairy, sugar, fried foods), or spleen qi deficiency impairing fluid metabolism. Key signs: heavy sensation in the body, edema, turbid discharge, greasy tongue coating, slippery pulse, feeling of fullness, sluggish bowels. Dampness often combines with cold (Cold-Damp) or heat (Damp-Heat) to form mixed patterns.

What is dampness subtypes and protocols?

1. Cold-Dampness (Hanshi)

Key symptoms: Joint pain with fixed heavy sensation, worse in cold/wet weather, edema in lower limbs, clear scanty urine, thick white tongue coating.

Protocol: ST36 (Zusanli) 15-20 min (strengthens spleen to resolve damp), GV4 (Mingmen) 15-20 min (warm yang to evaporate damp), BL23 (Shenshu) 15 min, SP9 (Yinlingquan) 10-15 min (influential point for dampness), ST40 (Fenglong) 10 min (resolves phlegm-damp). Moxa box on lower back for 20 min. Treat 3-4 times weekly.

2. Spleen Deficiency with Dampness (Pixu Shi Sheng)

Key symptoms: Poor appetite, abdominal distension, loose stools, fatigue, heavy limbs, edema, overweight.

Protocol: ST36 (Zusanli) 15-20 min, CV12 (Zhongwan) 15-20 min, SP6 (Sanyinjiao) 10-15 min, SP9 (Yinlingquan) 10 min, ST40 (Fenglong) 10 min, BL20 (Pishu) 10-15 min. Treat 3 times weekly for minimum 12 weeks for chronic cases.

What is evidence summary?

Clinical research supports moxibustion for dampness patterns. A 2018 study found ST36 and SP9 moxibustion reduced lower limb edema by 28% in patients with spleen deficiency-dampness. A 2020 systematic review reported that ST40 (Fenglong) moxibustion significantly reduced phlegm production in bronchitis patients with phlegm-dampness pattern (73% effective vs 45% control). For weight management related to dampness retention, a 2021 RCT showed CV12+ST36 moxibustion reduced waist circumference by 3.2 cm more than diet alone.

What is contraindications?

  • Pure Damp-Heat with fever, thirst, yellow tongue coat – use moxa sparingly or at distal points only
  • Edema due to heart or kidney failure requires medical supervision
  • Standard pregnancy precautions
